It does not give a city and state, just 709 Edmond St. below the name. Yeast Foam Malted Milk is advertised on the front and back covers, along with the front and back inside covers. The inside features two hooks to place a paper menu of the soda fountain offerings. The menu has plastic covers and metal outer edges for lasting longevity. A small tear can be seen on the upper left of the front cover. This menu measures 9.25 inches tall by 6.25 inches wide. 20228-EPP
